Providers of high-risk AI systems on the EU market must report any serious incident to the market surveillance authority of the Member State where the incident occurred, immediately and within set deadlines (15 days; 2 days for serious harm to critical infrastructure or death; 10 days for widespread infringement of fundamental rights).
